Prevention Services Survey Data Due December 4
Dear Superintendents and School Leaders,
As a reminder, on October 19th public school districts, community schools, STEM, Joint Vocational School Districts, and chartered non public school leaders received the Prevention Services Survey to be completed for each building in their district. House Bill 166, section 3313.6024, requires Ohio school districts to report the types of prevention-focused programs, services and supports including substance abuse, suicide, bullying, and other harmful behaviors, for each grade (kindergarten-grade 12) in each building.
Superintendents received an email with a secure link to access the district survey page. From the district page, each building can submit its own survey report. The survey link should be forwarded to the building principal to be completed by professional staff in the building who have knowledge of the prevention-focused programs, services and supports provided in the school.
All districts and schools must respond to the Prevention Services Survey between Oct. 19 and Dec. 4, 2020. The Prevention Services Survey PowerPoint training provides further directions for completing the survey. The district’s completion of this survey fulfills the reporting requirement for HB 166. Please submit data from the 2019-2020 school year by close of business on Dec. 4, 2020. This survey was delayed because of ordered school closure due to Covid-19.
